Advance Practice Nurse (APN) roles as a Change Agent:

Advance Practice Nurse (APN) is an additional training for nursing. Nurses, who take additional training for APN will become certified for checking and treating patients without the consent and help of a doctor. This is a huge shift of role from nurse to advance practice nurse.

Advance practice nurses can work in different clinics, medical offices, hospitals and in nursing homes. There are four types of certifications for nurses to shift from nursing category to advance practice nurse which includes nurse practitioner (NP), certified nurse midwife, nurse anesthetist and clinical nurse specialist.

Nurse practitioner (NP) can see the patients out of any medical offices and hospitals without the help of any doctor. NP holder nurses can take care of patients for regular medical checkups. NP holder nurses work as a primary health care providers who check patients prior to the doctor. They also perform several routine diagnostic tests which is a part of regular checkups.

Certified nurse midwives are the prenatal care providers for expecting mothers in their pregnancy. They can also provide prenatal care in maternity homes by delivering the babies as well.

Nurse anesthetists assist the patients in the surgical operating room by helping the doctors during the surgery. Here the work for APN is to administer medication for patients related to sleeping and pain and to monitor the signs of the surgery like heartbeat and other signs.

Clinical nurse specialists provide specialized care services. They can make decisions related to the diagnostic of patients and about their medical care as well. Clinical nurse specialists are general practitioner nurses who provide a wide range of medication services and medical care.

An example for acting as an Advocate:

A while ago, I experienced a situation where I met a patient with his family. They  belonged to a poor family and their only son had the Cushing’s syndrome with type II diabetics at the age of 35 and the medication prescribed by the doctor were very costly. By the profession of Nurse, I am acknowledged about the variety of the resources from where they can get the medicines with same ingredients/formulas at low cost.

In this situation, I assisted the patient and his family who were facing a difficulty in paying the cost for prescribed medicines and other related services. This is the part of my duty and according to the certified holder of advance practice nurse, in this situation I can assist and provide medication and treatment like a doctor.

Implementation of Advanced Practice Nurse (APN):

There are nine step for the implementation of advanced practice nurse (APN) role at the organization, state and national level. These steps are mentioned below: (David Old en burger, 2017).

  1. This step improves the patient’s health by providing the human resources and financial resources for nursing department in order to advance the worldwide health.
  2. Second step for implementation the advance practice nurse in organization, state and national level is to identify the stake holders by developing the Pan American Collaborative Network.
  3. Third step is to determine the unseen patients health needs by making the academic and research intuitions with the support of national task forces (NTF).
  4. In fourth step of implementation, there is a need to determine the priorities and goals in order to introduce the advance practice nursing (APN) in the organization for the provider of primary health care.
  5. Step five involves the identification of different strategies in order to achieve the goals and priorities made in step four. Every country as well as every organization is responsible to make the advance practice nursing roles and responsibilities for its public and employees.
  6. Step six reflects the implementation of the strategies that are identified in the above step.
  7. This step includes the creation of the body which evaluates and monitors the implementation of plan and strategies.
  8. In this step, above making body will evaluates the roles and responsibilities for the implementation of advanced practice nursing (APN) in an organization, state or national level.
  9. Last step includes the long term monitoring of the roles and responsibilities of the advanced practice nurse (APN).

Credibility of Advocacy:

By analyzing the past two decades, credibility of advocacy is the much debated topic in nursing profession and advanced practice nursing (APN). Moreover, advocacy is the most important and critical role and value in the nursing but it findings are limited in practice. In worldwide, variety of studies have been made in order to find the barriers in nursing advocacy but yet it has not been totally identified.

A study made in 2006 on the barriers and facilitators of patient advocacy found that by making the data analysis, nurses identified the ineffective, lack of back hand support, law avoidance, low code of ethics sand motivational level, incomplete communication with patients, royalty to peers and lacking of time to interact with patients and their families lead to the barriers in advocacy (Reza Negarandeh, 2006).

These factors occur due to low nurse patient relationship, unrecognition of patient needs, less clarity of the nurses’ responsibilities and knowledge as well as skills that are insignificant in adopting the advocacy role and values in nursing.............
