Abstract:
This case reveals how Fiona Jenkins, Head of Physiotherapy for South Devon Health Providers, evaluated and revamped the stroke service care path. She had simply 4 months to provide a proposition and then execute it in accordance with the National Service Structure (NSF) standards. Her difficulty was to collaborate the various stakeholders - main care, secondary care and social services.
