Miscommunication:

When Matty was facing unstoppable seizures; the relevant Doctor was not well informed about the situation and was getting information quite late, which was the reason why the doctor was not able to make appropriate decisions at the right time.

  • Risk: If people are not freely able to communicate the problems;the solutions will get delayed, making it difficulty to cater the patient’s needs because of the confusions created by the miscommunication or lack of communication.
  • Solution: Make people comfortable with each other and ask them to inform right on time to the relevant person, instead of waiting till the last moment.

Missing Post-Surgery Plans:

            After the surgery; the staff handling the patient was not aware of the situation and there were no particular guidelines mentioned regarding the proper care of the patient so they were unable to handle the emergency situation.
