Depressive Disorder Case Study Solution

Diagnosing Patient

For diagnosing the patient who is suffering from depressive disorders, I would conduct a physical exam by focusing on the neurological and endocrine system. The physical exam would consist of asking questioning regarding his family background who had similar depression symptoms to identify whether he had faced the same kind of symptoms before losing his job or whether the patient has a chronic depression(MM, 2016). Other diagnosing steps would be through conducting lab tests byanalyzing the patient’s blood. A psychiatric evaluation by asking the patient on filling the questionnaire would help in giving more details regarding the patient’s symptoms, thoughts, feeling and behaviour.

Medication class indicated

After diagnosing Frank who has been feeling lack of energy, motivation and trouble sleeping, he has been identified with the major depression. The medication classes which have been identified according to the diagnosis are the following:

  • Doxepin
  • Clomipramine
  • Prozac
  • Citalopram
  • Xanax

One of the antidepressant drugs which arementioned above is Prozac which is one of the commonly used drugs worldwide for the treatment of depression. The drug consistsof various forms such as liquid and tables. It is normally instructed by the doctors to take 20mg of Prozac per day at first. It is used for treating manyillnesses regarding mental disorders such as:

  • Panic Disorder
  • Bulimia nervosa
  • Obsessive-compulsive disorder
  • Depression

The contradictions for medications, in this case, would be the use of strong medications SNRI and MAOI that may contain high side effects.

Neuro circuitry responding with the symptoms

Neuro circuitry refers to the system of neuro circuits which helps in controlling and regulating the neural activity. The symptoms which have been identified are lack of energy, demotivation and trouble sleeping have been responding with the neuro circuitry which is causing mood disorder, not engaging in any other activities, social problems and affecting intellectual skills.

Recommending medication

Since the patient is afraid of taking medication this may impact the relationship with his wife. The first treatment procedure for the depression disorder would be through therapy. The patient had lost his job and is having a hard time finding another one which has caused lack of energy and demotivation. Since he had stopped seeing his friends this has also increased his stress and social anxiety. He also had the responsibility of keeping his wife happy which also burdened him. The therapy may help frank in modulating the neuro circuitry to remit these symptoms by boosting his motivation, confidence and helping in overcoming his stress, depression, anxiety and social problems.

Medication Alternative

If the therapy is ineffective for solving Frank’s mental disorder, the next medication which I would consider is prescribing Citalopram which is one of the commonly used antidepressant drugs. Since Frank was afraid that the medication would affect his relationship, I would prescribe in taking less dosage which would cause fewer side effects. Citalopram has fewer side effects compared to other antidepressant drugs and is considered a good antidepressant for treating depression and sleeping disorders.

Rationale

Citalopram is likely to modulate then euro circuitry to remit these symptoms since this antidepressant is known and commonly used for treating depression and sleeping disorders. This drug class would help in improving mood disorder, social anxiety and intellectual skills.

Literature Review

The selective serotonin reup take inhibitors (SSRI) consist of a variety of antidepressant drugs for treating depression and anxiety. One of the SSRI is the Citalopram which is recommended as an alternative medication to Frank for treating his depression. This research purpose is to study and evaluate the effectiveness of Citalopram for the patient suffering from mental disorder. The study had been conducted on 23 psychiatric and 18 primary care in which it was found that Citalopram was effective against the patients (Trivedi, Rush, & Wisniewski, 2006)....................
