Introduction

This case isabout75 year oldperson who had a history of chronic obstructive disease due to being a heavy smokerand wasadmitted to the hospital for his surgery being conducted for the coronary artery bypass graft. After his surgery, he had a number of asetback in which the hospital's doctors and nurses were not able to remove him from ventilation. Ever since his surgery, he wasfrequentlysuctioned and was being carefully monitored if he develops ventilator-associated pneumonia which is one of the lung infection and is caused by the ventilation mechanical breathing.

The patient secretions had been becoming thick in which one of the experienced nurses had to instilsaline in the patient’s endotracheal tube in order to loosen the secretion. This practice had no longer been acceptable as most doctors considered this as a risky procedure. However, the experienced nurse who had been working on the critical care believed that this wasaneffective procedure for managing a person’s secretion while being ventilated.

Strategies for searching evidence

There are few strategies which would be used for searching evidence as forwhether the instilling of saline on a patient’s endotracheal tube for loosening his secretion while being ventilated is still considered a safe and acceptable procedure. The strategy is to analyze the past data which is related with the instilling of saline for loosening the secretion of the patient. The data would be gathered from the secondary sources which are available on the internet and other sources.

Since a lot of work has been done primarily on the particular topic from a different perspective, these studies will help us in developing the fundamentals of our research. Mostresearchers have carried out observation study in which several patients who havegone through coronary artery bypass are observed whether the instilling of saline on the patient’s endotracheal tube has a positive impact on the removal of  aperson’s secretion and improvement on the oxygen.

Summary of the evidence

In one of the research which is a study of whether the instilling of saline during the endotracheal suctioning having an impact on the secretion and oxygen. This study was a qualitative research which was conducted through observing around 35 patients who had been admitted to the hospital for the surgery of coronary artery bypass grafting. In the research two groups were formed in which the 35 patients have been divided among them. One of the group would have to instill saline through theendotracheal tube while the other group would not have to use saline(D, 1999).

The patients were given around 5 millilitres of saline through the endotracheal tube for removing their secretion and improving the oxygenation. The time taken was more than 3.78 minutes longer than the other procedure. However, the results which had been founded were that the instillation of saline through endotracheal tube improves the oxygen of the person who is admitted in the hospital and is in ventilation due to their coronary artery bypass grafting surgery(D, 1999).

Another of the research had been carried out regarding the instillation of saline through the endotracheal tube which used on the pneumonia patients who are admitted to the hospital and are ventilated. For conducting the research, same techniques had been used which is through observation and the sample size on this research was of 16 patients with pneumonia. There were 3 methods of instilling saline to the patients which consisted of 0, 2 or 5 millilitres of saline...................
